Please go to your profile and answer several questions about your medical status.

Click the "Medical" section that is under the "MY MOBILIZATION INFO" tab.  Once there, please answer the questions by entering in the appropriate information in each box.  

PLEASE DO NOT LEAVE A BOX EMPTY

If the questions DO NOT apply to you simply enter "N/A"  in the box. This is important because it shows that you have read and understood the questions.  

**You must have all medical boxes completed (except for the 'Vaccinations Received" box) within ONE MONTH of your commitment date!!**

If your medical information changes for any reason, say you get put on a new medication or are suddenly diagnosed with an extreme case of Odontarrupophobia, PLEASE NOTIFY YOUR MOBILIZER!!
